University Scholarship
If you're working toward your bachelor’s degree, you could be eligible for up to $15,000 per year. If you are applying for a merit scholarship, you could be eligible for $1,000.

What You
Will NeedHere’s what you’ll need.
- Register with the Education Advantage program.
- Obtain official transcripts.
- Get the medical verification form filled out and signed.
- Write personal statement.
- Have both applicant appraisals ready to go.
- Sign and date the consent form.
- Get copy of financial award letter (if applying for a need-based award).
- Complete application form, including financial info section (if applying for a need-based award).
- Look it over again. Incomplete applications are disqualified.
- Submit!
Conditions
All need-based awards will be awarded on a "last dollar" basis, net of any other non-loan assistance received by the student. Scholarship America, as the program administrator, will work with the student and the educational institution each year to determine the appropriate amount of scholarship aid.
Scholarships provided by the program may be used for tuition, books, laboratory supplies, and mandatory fees on behalf of award recipients. Scholarships provided by the program will not cover room and board expenses.

Educational achievementHigh school senior, a high school graduate of any age, GED certified, or a current undergraduate/postsecondary student
Enrollment statusEnroll in a full-time undergraduate study at an accredited four-year college or university in the US for the entire upcoming academic year
Residency statusMust be a legal and permanent resident or citizen of the United States
Financial needNeed-based award: Must demonstrate financial need, as determined by Scholarship America
Merit scholarships: No requirement to demonstrate financial need
Academic achievementDemonstrated record of academic success. Have a current or most recent cumulative GPA of no less than 2.00 on a 4.00 scale
Personal statementApplicants must submit a personal statement addressing their story.
Letters of recommendationMust submit two (2) applicant appraisals. Recommendations should include the following:
- Academic recommendation (teacher, counselor, principal, etc.)
- Community recommendation (recommender should be familiar with your activities in the hemophilia community or elsewhere; cannot be a family member or involved in your medical care)
Medical Verification formMust submit a Medical Verification form confirming diagnosis of hemophilia A, including those with inhibitors*
Use of a Baxter product or service is not required for eligibility or consideration.
Lawful citizen/residentMust not have a felony conviction
*The form must be completed by the patient’s physician.

Renewal Criteria
Awards may be renewed up to three additional years or until a bachelor’s degree is earned, whichever occurs first.Criteria
Requirement
Academic performanceMust be in good academic standing, as defined by each school; if a school lacks this classification, a minimum GPA of 2.0 out of 4.0 will be used instead
Enrollment statusMust be enrolled full-time in an eligible program and institution
Health managementMust provide evidence of participation in annual comprehensive clinic and routine dental care. Compliance will be documented by a form to be completed and signed by the student’s physician and dentist
Community serviceMust perform and document 20 hours of community service in each year funding is received. Candidates are encouraged to volunteer in the hemophilia community but this is not a requirement. Service will be documented by a form to be completed by the student and signed by the leaders/organizers of the community service activities
Personal updateMust provide an essay (word count: minimum 250) detailing the following three topics:
- Academic progression
- Career goals/developments
- Value of funding assistance
Lawful citizen/residentMust maintain no criminal record
Contact informationMust maintain up-to-date contact information with the program administrator

Planning
Completing your Education Advantage program application doesn’t have to be a harrowing experience. Make a plan.
You will need to work with several people along the way, so be sure to start early. Call the offices below. Get contact info for a person at each office. Be sure to follow up. Also find out if there will be any fees you need to pay.
To complete your application you will have to contact:
- Your College Financial Aid Office
- Your Doctor’s Office
- Whomever has your Academic Transcripts*
- References to complete Applicant Appraisal Forms
- Your Parents for tax information

* Graduating high school students will need to send high school transcripts. College students need to send college transcripts from every college they have attended.
# Tax info is needed for the financial information section of the application. If you are applying for a Merit Scholarship, you don’t need to add tax info, but be sure to check the Merit Scholarship box in the financial section of the application.
